Authorization for Use and Disclosure of Your Information

AUTHORIZATION FOR SHARING YOUR INFORMATION WITH HTV

Please read the following carefully. This describes how Populus Media d/b/a Populus Health Technologies shares information about you to provide the services you have requested.

You have requested that Populus Media d/b/a Populus Health Technologies provide services to you, including matching you with a provider in your area or through a telehealth consult, and a pharmacy to fill any prescriptions issued by such provider (the "Services").

Information Covered. "Customer Data" means information about you that is collected by Populus Media d/b/a Populus Health Technologies in the course of providing the Services, including information you submit when requesting the Services, and information about your interest in certain medications and your health condition(s). Some or all of the Customer Data may be individually identifiable information subject to protection under state law.

Who Receives Your Information. "HTV" means Health Tech GmbH, the company that operates the SLYND you are requesting. HTV will act as the controller of the information it receives and will handle it in accordance with HTV's privacy policy, available at HTV Privacy Policy.

Your Direction to Share. With your permission, Populus Media d/b/a Populus Health Technologies will share your Customer Data with HTV.By acknowledging your authorization, you direct Populus Media d/b/a Populus Health Technologies to transmit your Customer Data to HTV on a one-time basis. HTV will use your Customer Data to provide and support the Services and the Product.

We Do Not Sell Your Information. Populus Media d/b/a Populus Health Technologies does not sell your Customer Data and is not paid in exchange for sharing your Customer Data. Populus Media d/b/a Populus Health Technologies is compensated by HTV for the platform, matching, and related services it provides. Once your Customer Data is shared with HTV, it will be handled under HTV's privacy policy and may be subject to laws different from those that applied while the data was held by Populus Media d/b/a Populus Health Technologies.

Voluntary. Populus Media d/b/a Populus Health Technologies will still provide the Services to you even if you do not sign this Authorization, except to the extent sharing your Customer Data with HTV is necessary to deliver a Service you have requested.

Revocation. Because your Customer Data is shared with HTV to provide the Services, any request to stop using or to delete your Customer Data must be directed to HTV at privacy@prio-one.com · HTV USA, Inc. d/b/a Prio One, 25 Kent Avenue, Brooklyn, NY 11249. HTV will handle your request under its privacy policy and applicable law. Once your Customer Data has been shared with HTV, Populus Media d/b/a Populus Health Technologies is no longer able to stop HTV's use of, or delete, that data.

Duration. This Authorization applies to the one-time transfer of your Customer Data to HTV described above. Because that transfer occurs at the time you request the Services, this Authorization cannot be revoked as to Customer Data already transferred; requests concerning transferred data should be directed to HTV as described under "Revocation."
